Vacancies:
We currently have 1 vacancy available in our purpose-built 2-bedroom villa, designed for participants with High Physical Support (HPS) SDA funding.
Level of Support:
• Fully accessible participants.
• Must have full accessible funding.
Age and Gender Suitability:
This property is best suited for male or female environment. Residents will be considered within the age range of 18–65 years old who have high physical support funding.
Types of Support Provided:
Participants can access a wide range of personalised supports, including:
• Medication management
• Positive Behaviour Support
• Meal preparation and grocery shopping
• Assistance with household tasks
• Appointment coordination and transport
• Personal care and activities of daily living, tailored to individual needs
• Fully accessible vehicle available on-site, supporting participants to attend appointments, access the community, participate in social and recreational activities, and achieve their NDIS goals
Sublime Care’s Villa is overseen by our Team Leader, Brooke, who is a Registered Nurse, providing strong clinical oversight and leadership. Our dedicated Clinical Care Educator, Hanna, also delivers ongoing training and competency development to our support teams, ensuring participants receive high-quality, person-centred care.
