Restorative Care Pathway (Support at Home)

Sometimes a short burst of the right support can make all the difference. When mobility changes, strength declines, or everyday tasks start to feel harder than they used to, the Restorative Care Pathway offers a structured way to regain confidence and improve function at home.

 

At Sublime Care, we provide restorative care that is goal-focused, coordinated, and delivered by a skilled multidisciplinary team. Our aim is simple: help you stabilise or improve your function, so you can continue living safely and independently at home for as long as possible.

What the Restorative Care Pathway can include

Restorative care provides targeted, short-term support designed to help you regain or maintain physical function. Each episode includes:

 

• Short-term, intensive allied health and nursing services
• Goal-oriented, structured interventions
• A coordinated multidisciplinary team approach
• A focus on improving, maintaining, or slowing the decline of function
• Dedicated funding separate from your ongoing Support at Home budget

 

Every restorative care episode centres around a personalised goal plan—your goals, your priorities, and what matters most to you.

Who this support is for?

The Restorative Care Pathway is ideal if:

 

• You’re experiencing changes in strength, balance, mobility, or daily function
• You’re motivated to participate in short-term therapy
• Your goals can realistically be achieved within 16 weeks
• You can continue living at home with little or no ongoing support afterwards

 

Restorative care may not be suitable if you are already receiving end-of-life support, transition care, permanent residential care, or if you have recently completed two restorative care episodes in the past 12 months.

How it works at Sublime Care

Start with a friendly conversation
We take time to understand what’s become more challenging and what improvements matter most to you.

 

Build a personalised goal plan
You’ll work closely with your restorative care partner to set clear, achievable goals. This plan guides the entire episode.

 

A multidisciplinary team working together
Physiotherapists, occupational therapists, nurses, and other allied health professionals collaborate to deliver targeted supports. Your restorative care partner coordinates everything and ensures your services stay aligned with your goals.

 

Up to 16 weeks of focused support
Each restorative care episode provides approximately $6,000 in dedicated funding (or up to $12,000 for eligible participants). This covers intensive clinical services designed to improve your function.

 

Clear communication and ongoing monitoring

We review your progress regularly, adjust goals as needed, and ensure the team is working cohesively.

 

Thoughtful transition planning

At the end of the episode, we prepare a detailed exit plan outlining progress achieved, supports delivered, and recommendations for next steps so you feel ready, confident, and supported moving forward.

Your Restorative Care Team

Our multidisciplinary team may include:

 

• Physiotherapists
• Occupational Therapists
• Exercise Physiologists
• Registered Nurses
• Other allied health professionals as required

 

Depending on your needs, we may also collaborate with your GP or specialist for clinical oversight (accessed via Medicare or other programs).
