End-of-Life Pathway (Support at Home)

When someone chooses to spend their final months at home, the right support can make all the difference. Sublime Care’s End-of-Life Pathway helps older people remain where they feel safest and most comfortable — at home — with practical assistance, respectful care, and a coordinated approach that works alongside specialist palliative care services.

 

This support is designed for people who have a life expectancy of around three months or less and want to stay at home with dignity, familiarity, and as much comfort as possible. We provide fast access to essential services and clear communication so families feel supported, not overwhelmed.

What the End-of-Life Pathway can include

End-of-Life support focuses on the daily living tasks and practical assistance that bring comfort and reduce stress during a sensitive time. Depending on the participant’s support plan, services may include:

 

• Personal care (bathing, dressing, grooming, toileting)
• Meal preparation and nutrition support
• House cleaning and household assistance
• Respite support for carers
• Social and emotional companionship
• Assistance with maintaining household routines
• Access to low- and medium-tier assistive technology where appropriate

Who this support is for?

The End-of-Life Pathway is for older people who:

 

• Wish to remain at home during their final stage of life
• Have been assessed by a doctor or nurse practitioner as having a life expectancy of three months or less
• Have an Australian-modified Karnofsky Performance Status (AKPS) score of 40 or below
• Need short-term, intensive home-based supports to stay safe and comfortable

 

This pathway supports both new and existing Support at Home participants.

How it works at Sublime Care

Start with an urgent referral
Because time matters, the process begins with a completed End-of-Life Pathway Form filled out by a doctor or nurse practitioner. Existing Support at Home participants can request an urgent Support Plan Review to transition quickly.

Our team connects with the participant and their family to understand preferences, routines, cultural needs, and what will bring the most comfort.

We create a plan that is practical, gentle, and aligned with what matters most — comfort, safety, familiarity, and dignity.

Your dedicated Care Partner coordinates all supports, liaises with palliative care teams, and ensures communication between everyone involved remains smooth and clear.


Flexible support as needs change

We adapt services as the person’s needs evolve, ensuring support remains responsive, appropriate, and respectful.

 

Care Management

Every person on the End-of-Life Pathway receives dedicated care management, ensuring:

 

• A clear and personalised care plan
• Coordination with medical and palliative care teams
• Anticipation of changing needs
• Smooth communication with family and supporters
• Oversight of all service delivery and documentation

 

There is no cap on care management costs; they must simply be reasonable, agreed upon, and focused on the participant’s needs.

Exiting the Pathway

A person exits the pathway when:

• They choose to stop receiving home-based end-of-life supports
• They are no longer able to remain at home safely
• The funding period ends
• They pass away

If more long-term support is needed, Sublime Care can request a Support Plan Review to ensure ongoing services are in place.

When a participant passes away, families simply notify us — we take care of the required processes with sensitivity and respect.
