As we get older, the home we love sometimes needs small adjustments or the right equipment to stay safe, comfortable, and easy to navigate. The Assistive Technology and Home Modifications (AT-HM) scheme helps older Australians access the items and home improvements that support independence without using up their regular Support at Home budget.
With Sublime Care, you get clear guidance, coordinated support, and practical solutions that fit your needs, your home, and the way you like to live.
AT-HM funding can support a wide range of items and changes designed to make daily life easier, safer, and more manageable. Depending on your needs and assessment, support may include:
Assistive Technology
• Walking sticks, walkers, or wheelchairs
• Shower chairs, bath boards, or toileting aids
• Bed or seating supports
• Transfer equipment
• Personal alarms or safety devices
• Everyday items that make tasks easier
Home Modifications
• Grab rails in bathrooms, hallways, and around the home
• Internal and external handrails
• Ramps or threshold ramps for easier access
• Stair lifts
• Bathroom redesigns or walk-in showers
• Widened doorways or hallways for mobility access
These supports help reduce falls, improve mobility, and create a safer and more accessible home environment.
Assistive Technology & Home Modifications are ideal for older people who:
• Want to stay living safely and independently at home
• Find certain areas of their home harder to move around or manage
• Need equipment to make daily tasks easier
• Require home adjustments to reduce risks or improve mobility
• Are recovering from illness, injury, or changes in physical function
Start with an aged care assessment
During your assessment, the need for assistive technology or home modifications is identified. You may receive approval for:
• An Assistive Technology funding tier
• A Home Modifications funding tier
• Or both

Assessments and recommendations
If required, a qualified allied health professional will assess your needs and recommend the right equipment or modifications.

Flexible support over time
Most people have 12 months to use their funding. Those with progressive conditions may automatically receive 24 months, with the option to extend up to 48 months.
