As a registered NDIS provider, we aim to provide a variety of community care and support services that would help participants develop foundational skills and encourage them to participate actively in the community. Our Community Services team provides support in the following areas:
Our High-Intensity Supports:
As a registered High-Intensity Support provider, Sublime Care excels in delivering complex care solutions under the guidance of our experienced team:
– Complex Bowel Care
– Enteral Feeding and Management (including PEG feeding)
– Urinary Catheter Care
– Diabetes Management and Subcutaneous Injections
– Day-to-Day Medication Management
Our support staff can provide these services as they have additional training from a Registered Nurse (RN) and recognised health professionals. This safeguards participants as the staff can skillfully perform clinician tasks within a community setting and have the knowledge, skills and experience to escalate any clinical situations required to promote positive health outcomes for our participants. Sublime Care provides in-house, personalised training for our support staff, and we encourage participants, their circle of support and stakeholders to be involved in this process wherever possible.
Community Care as your NDIS Core Support Choice
Our services are a variety of support that helps a participant’s daily living activities. It falls under the category of NDIS Core Support Budgets that may be funded in an NDIS plan that aims to provide individual support to participants with everyday activities, support their current disability-related needs and work towards their goals.
