We welcome referrals!
Sublime Care is now accepting referrals for Allied Health and Domestic Assistance services for participants who are under Commonwealth Home Support Programme (CHSP) funding! We currently have the capacity to provide Domestic Assistance, Speech Pathology, Occupational Therapy, and Physiotherapy.
