Sometimes the smallest changes make the biggest difference. For Trish, a resident in South West Sydney, a new access ramp meant the freedom to do her own washing, reach her car independently, and live life on her own terms. This is the story of how Sublime Care’s Occupational Therapy team made that happen.
Trish’s Journey
Trish was referred to Sublime Care’s Senior OT, Aimee, through the Commonwealth Home Support Programme (CHSP) via My Aged Care. The goal was simple. Assess Trish’s home environment and identify what modifications and equipment could help her live more safely and independently.
Aimee conducted an initial assessment on 4 November 2025, followed by a joint site visit with builder Stuart from BaptistCare Home Modifications & Maintenance Service on 17 November 2025. Together they assessed the property and developed a clear plan of action.
The Home Modification
Aimee completed a full OT home modification report, also known as a scope of works, outlining the specifications for a new access ramp at Trish’s home. The ramp was completed in early 2026 by the skilled team at BaptistCare, and the results were nothing short of impressive.
Trish had this to say about the finished ramp:
“Baptist Care has completed my ramp. They did an excellent job. Very impressive. It would still be standing after the house fell down I think. These two carpenters and the concreter are to be commended on their work.”
And the impact on her daily life has been life-changing:
“I’m delighted with the ramp and it makes it easy for me to do my own washing instead of depending on Linda. I can also reach my car more easily. Baptist Care did an excellent job and I recommend them.”
Additional Equipment Through Get2Go
Alongside the home modification, Aimee was also able to source a range of assistive equipment for Trish through the Get2Go equipment program, including:
- Personal care aids
- Adaptive cutlery and utensils
- A kettle tipper to support safe and independent meal preparation
Each of these items was carefully selected to support Trish’s ability to manage her Activities of Daily Living (ADLs) with greater confidence and independence.

This outcome is a powerful reminder of what occupational therapy can achieve. It is not just about equipment or modifications. It is about restoring confidence, dignity, and independence for the people we support.
