What is CHSP?

The Commonwealth Home Support Program (CHSP) is a government-funded initiative dedicated to helping seniors aged 65 and over maintain their independence and enhance their quality of life at home. Our CHSP services provide personalised support tailored to the unique needs of each individual, assisting with daily tasks in a way that fosters comfort and confidence. How the CHSP Works?

The Commonwealth Home Support Program (CHSP) provides entry-level support for seniors needing assistance with one or two services to maintain independence at home. CHSP eligibility is based on age and level of need, with funding options available for both government-subsidized and private services.

Eligibility for CHSP

To qualify for CHSP, individuals must meet the following criteria:

 

Age:

• 65 years or older (50 years or older for Aboriginal or Torres Strait Islander people).

• 50 years or older (45 years or older for Aboriginal or Torres Strait Islander people) and on a low income, homeless, or at risk of homelessness.`1

 

Level of Need: Assistance may be required for:

Low-level ongoing needs.

• Short-term support after an injury or illness.

• Interim support while waiting for a Home Care Package (HCP).

• Assistance due to risk of homelessness.

Seniors needing 1-2 entry-level services. Ongoing low-level needs, short-term recovery, or interim support.

Government Subsidised

Ideal for those requiring light support at home.

Home Care Package (HCP)

Those needing more comprehensive, ongoing support than CHSP can provide.

Government Subsidised

Suitable for a broader range of care needs, from basic to higher levels.

Home Care Package (HCP)

Available to anyone, regardless of government eligibility, or for those who prefer to pay privately.

Non-Subsidised (Full Cost)

Flexible support options for those awaiting funding or who exceed funding limits.

In addition to CHSP and HCP, short-term government-funded care options are also available for those recovering from illness or injury, and specific programs are provided through the Department of Veterans’ Affairs (DVA) for eligible individuals.

What's the Process?

All applications for government-funded aged care services are managed through My Aged Care. If you are eligible for CHSP, you can choose Sublime Care as your authorised provider. We will work closely with you to coordinate personalied home care services tailored to your needs.

Step 1: Apply for CHSP

To begin, apply through My Aged Care by completing a quick online form or calling 1800 200 422. This application determines your eligibility for government funding based on factors like age, income, and level of need.

Step 2: Describe Your Support Needs

If your needs are low-level, the Regional Assessment Service (RAS) will contact you to arrange an in-home assessment for CHSP. For more extensive support, the Aged Care Assessment Team (ACAT) will reach out to discuss options, including the Home Care Package (HCP).

Step 3: Complete Your Face-to-Face Assessment

Your RAS assessor will visit your home to better understand your daily routine and goals, identifying areas where you need help. This assessment forms a personalised support plan, designed to match your specific needs.

Step 4: Get Approved and Receive a Referral Code

Once approved, you’ll be provided with a unique referral code to access CHSP services. Just share this code with Sublime Care, and we’ll guide you through the next steps to start your care.

Step 5: Set Up Your Care Agreement

We’ll meet to discuss your goals, preferences, available services, and any costs. After reviewing your options and creating a care agreement you’re comfortable with, your services can begin.

Step 6: Ongoing Care and Support

Each month, you’ll receive a summary of services and costs. Your Care Coordinator will stay in close touch to adjust services as needed, ensuring you’re always receiving the best support for your lifestyle.
