Mental Health Conditions are often overlooked health concerns in Australia. At Sublime Care, with the support of NDIS funding, we are dedicated to shining a light on this condition that is too often stigmatised. It is estimated that in Australia, approximately 3 million people experience mild to moderate mental health issues, while around 700,000 individuals have severe and persistent mental health conditions that require support due to their disability.

Our Mental Health Support Services

We offer a range of mental health support services to meet the unique needs of participants. Our services include:

Core Supports (Daily Living): Assisting with activities of daily living, such as personal care, household tasks, and meal preparation, to promote independence and well-being.

High-Intensity Support: Providing intensive assistance and guidance for participants with complex needs where a skilled or more experienced support worker may be required to deliver support.

Community Participation: Encouraging active engagement in the community through social activities, skill-building programs, and access to community resources.

Support Coordination: Assisting participants in navigating the NDIS system, coordinating services, and creating personalised support plans.

Accommodation Services: Offering suitable housing options and support for participants whose goal is to live independently.

Our Approach

1. Emotional Regulation: Our team members and support workers will receive on-going training to assist participants in identifying and managing their emotions effectively. They provide guidance to promote emotional well-being and resilience.

2. Cognitive Behavioural Techniques: We will employ evidence-based cognitive behavioural strategies to help participants challenge negative thoughts, develop positive coping mechanisms, and improve their overall mental outlook.

3. Psychoeducation: We believe in empowering people through education. Our team offers valuable information and resources about mental health conditions, helping participants and their families gain a better understanding of their condition and how to navigate daily challenges.

4. Social Engagement: We recognise the importance of social connections for mental health. Our workers support participants in building and maintaining meaningful relationships, fostering a sense of belonging and reducing feelings of isolation.

5. Goal Setting and Monitoring: Our collaborative approach involves working closely with participants to establish personalised goals and milestones. We provide ongoing support and monitor progress, adjusting strategies as needed to ensure continuous improvement.
