Sublime Care provides community-based supported housing and lifestyle support to NDIS-eligible. We offer a variety of NDIS accommodation services for participants who need help with and/or supervision of their daily tasks and goals. The goal of this service is to support the participant in developing their skills to live independently. Participants and their families can discuss their home and living goals as part of their planning conversation

These supports focus on
building capacity in independent living skills of all life domains, with
particular emphasis on the following areas ordinary to every individual’s life:
• Assistance in managing budgets and money
• Establishing life routines that promote positive outcomes for individuals
• Assistance to gain greater independence with household tasks such as cleaning, meal preparation and cooking meals
• Support with taking medications and living a healthy lifestyle, including support with medical appointments
• Supporting with planning, organisation and completion of daily tasks

What
makes OUR SERVICES different?
• We place significant importance on matching individuals with suitable housemates. We achieve this by including participants, their circle of support and stakeholders in the decision-making process of finding the right home with Sublime Care.
• Meet and greets are encouraged for all new participants; this ensures a more prolonged, meaningful social interaction is accessible for all involved to get to know the other person and identify any barriers in the potential match of housemates.
• We encourage participants to have a say and provide preferences relating to the support staff working in our Accommodation Services. This might include providing choices for the excellent support staff, assisting with interviewing, inducting new staff and providing ongoing feedback on suitability.
• We encourage smaller ratios of support, wherever possible, to promote more personalised care.
• We run regular participant house meetings, where each individual has the opportunity to provide input into the general running of their house, provide feedback and have open discussions with all parties.

Sublime Care provides an obligation-free consultation in the comfort of your own home, with the purpose of listening and understanding your needs. We’ll give you the best choices for your needed support, which you can freely explore, choose and control.
Sublime Care provides Accommodation Services in the following areas: Hunter Region, Central Coast, Newcastle and Sydney.
