Sublime Care offers Support Coordination services for participants who want to fully maximise their NDIS plan. This service gives participants the autonomy to personalise and tailor-fit the support that they will receive according to their needs and requirements

Support coordination helps build your ability to connect with informal, community and funded supports enabling you to get the most out of your plan and achieve your goals. This support will assist you to build the skills you need to understand, implement and use your plan. A support coordinator will work with you to ensure a mix of supports is used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently, and be included in your community.

How will Sublime Care’s Support Coordinator assist you:

Once you are funded for NDIS Support Coordination, Support Coordinator will connect you with various supports and services in the community and in the Government as per your NDIS Plan. Our role is to help you put your NDIS Plan into action and to the optimal level.   

Our Support Coordinators will:

• Help you understand funding and services that are made available in your NDIS Plan

• Help you understand how you can use your NDIS plan to meet your goals

• Help you understand how to use the NDIS portal so you can monitor how your funds are being utilised

• Introduce you to support options and providers and help you choose your preferred support/provider

• Negotiate your preferred services and prices, as well as develop service agreements and create service bookings with preferred providers

• Work with your circle of supports (family, GP, therapists, friends) in order to meet the goals of your individualised NDIS Plan

• Monitor the progress of your services and outcome of achieving your goals

• Assist you in preparing for your next NDIS review meeting

What we can offer:

Our qualified, accredited, and committed team of support coordinators provides two different levels of support for our participants:

Support Coordination: Coordination of Supports: 

Coordination is suitable if participants receive multiple or complex supports from a range of providers. The intended outcome of providing Coordination of Supports is to sustain a participant’s capacity and ability to design, develop and implement their supports. This includes, but is not limited to:

1. Connection: Assist the participant to develop knowledge, experience, and connections with the community and broader systems of support.

2. Support Design: Works together with participants to understand plan funding and its purpose. The support Coordinator will understand the participant’s confidence and skills and helps the participant identify what they want from services. Will develop and design support solutions to meet participant outcomes.

3. Establish Supports: Assist the participant to identify and consider support options and link the participant to the broader systems of support. Where practical create a support and action plan to facilitate the participant to implement their plan.

4. Crisis: Assistance to resolve points of crisis and develop capacity and resilience in the participant’s network.

5. Coach, Refine, Reflect: Coach the participant through challenges that come up. Helps participants prepare for review and report on achieved participant outcomes.

Our Onboarding Process

The Business Growth Team will support you through the onboarding process at the beginning. They will check with the Support Coordination Team to see if they have the capacity and skills to meet your needs. Once the documentation work is complete in compliance with Sublime Care, they will then contact the participant/carer and organise a meet and greet with the support coordinator to meet the participant, sign paperwork, and develop an individual support plan. Following the meeting, the support coordinator will be ready to begin providing services to the participant.

Sublime Care provides an obligation-free consultation in the comfort of your own home, with the purpose of listening and understanding your needs. We’ll give you the best choices for your needed support which you can freely explore, choose and control. 

As a registered NDIS service provider, we provide Disability services in the following areas: Hunter Region, Central Coast, Newcastle and Sydney. If you do not see the location where you are looking for a Community Care service, we encourage you to contact us to see if we could coordinate with a community care provider in the location of your choice.
