Our Behaviour Practitioners are highly skilled professionals dedicated to delivering effective Positive Behaviour Support. With extensive experience in the NDIS sector and backgrounds in areas such as education and disability services, they bring a deep understanding of diverse needs and evidence-based strategies. They excel in critical thinking, problem-solving, and building supportive relationships with individuals, families, and stakeholders. Committed to ongoing professional development and guided by the PBS Capability Framework, our practitioners ensure a person-centred and holistic approach to promoting quality of life and empowering individuals.
A Functional Behaviour Assessment is completed to understand what the participant is trying to communicate, what is influencing them and why they are using that behaviour. The assessments are conducted using evidenced based assessments and include interviews/discussions with the participant, their circle of support and their whole support team. This assessment is then used to build an individualised Positive Behaviour Support Plan (PBSP)
An Individualised Positive Behaviour Support Plan is then built which aligns with the person goals and the challenges identified in the FBA. The plan provides on the history of the participant and how they like to enjoy their time. It outlines skill building interventions that support the persons interests, that will improve their quality of life, and provide families and the support network strategies and interventions to address the behaviours of concern and have improved outcomes.
It provides teaching strategies to support skill building (including helpful behaviours and daily routines) and how to support the participant when they are using a behaviour of concern ( Self Soothing techniques and education to the support network to improve their understanding, support and interventions) , which aims to keep the participant and their support team safe and return to the participants baseline as quickly as possible.
Sublime Care prioritises comprehensive training for the participant, their circle of support, the support staff and all other providers to ensure participants have confidence in the skill level of their support team. Sublime Care will support the participant, circle of support and support network when a behaviour has occurred to debrief and assess what needs to be achieved to support the positive change the person is working towards. This will be achieved through group or individualised training, face to face, on teams, through emails and phone interations.
Sublime Care’s Positive Behaviour Support Practitioners will ensure all strategies are evidence-based and result in meaningful, measurable outcomes. These measurements and outcomes are monitored regularly and after a behaviour has occurred and will inform Positive Behaviour Support Plan reviews to reflect the changes needed including when a behaviour has decreased or been eliminated.
A restrictive practice is defined as any practice or intervention that has the effect of restricting the rights or freedom of movement of a person with disability, with the primary purpose of protecting the person or others from harm.
Sublime Care Positive Behaviour Support Practitioners will review any restrictive practices being used and support the Participant and their support network to identify these and document them in the Positive Behaviour Support Plan and create a plan to reduce or eliminate these and train the support network of the restricted use of these. Sublime Care will support the participant and their support network to have these authorised through the appropriate governing body where they cannot be eliminated.
