How to Make an NDIS Access Request?
If you believe you meet the NDIS requirements, you can submit an Access Request in one of two ways:
1. Verbal Access Request (VAR): You can make your request over the phone.
2. Paper Access Request Form (ARF): You can fill out a physical form.
To get started, you will need to provide age and residency evidence. The easiest way to do this is by giving the NDIA (National Disability Insurance Agency) consent to use your Centrelink record, if you are a Centrelink customer.
Additionally, you may need to provide evidence regarding your disability and how it impacts your daily life. This can be done by asking your treating health professional to complete the ARF – Part F, or by submitting an NDIA Supporting Evidence form. Supplying copies of any existing reports or assessments can also be helpful.
Regardless of how you choose to submit your Access Request, you will need to provide the NDIA with the same information and supporting evidence.

The NDIS Access Request Form (ARF) is a crucial document that you need to complete if you wish to apply for NDIS support. This form collects detailed information about your age, residency, and the impact of your disability on your daily life. It’s important to fill out the form accurately to ensure that your application is processed smoothly.
The ARF includes several sections, with Part F being specifically for your treating health professional to provide evidence of your disability. This section is vital as it demonstrates how your condition affects your ability to perform everyday activities. Additionally, an NDIA Supporting Evidence form can be submitted alongside the ARF to strengthen your application.
