How Community Participation Works

Sublime Care strongly encourages participants to actively engage in the community, which is crucial for physical, social, emotional, and intellectual development. Community engagement can be meaningful and is important to feel a sense of connectedness to the world around you.

• Sublime can support you to attend/participate in:

• Community Events or Shows

• Hobbies and leisure activities such as the Cinema!

• Cultural/ religious meetings, gatherings, or worship

• Medical Appointments

• Support programs such as swimming lessons, gym, or sports

• Shopping

• Community Centre

• Eating out

These supports are examples of community, social or recreational activities. These may be provided in a centre or in community settings at standard or higher intensity rates. We ensure that each event or activity is aligned appropriately depending on the participant’s individual needs.

Community Participation as Core Support or Capacity Building in an NDIS Plan

A participant’s NDIS Plan may contain one of two forms of community participation funds. Funding for community engagement that falls under the category of “Core Supports” can contract a support worker to assist participants in engaging in social, professional, and community activities community participation under “Capacity Building” includes support with increased social and community participation. This type of funding is used for training and development activities that assist individuals to integrate into the community and gain independence.

How the NDIS Supports the People for Community Participation

The NDIS actively engages and connects with people in local communities across Australia to make sure that they are sharing relevant and timely information about the Scheme and what is changing. They actively coordinate with people around the country, at NDIA information and consultation meetings, disability expos, in person and online. The feedback that NDIS gather is used to help stakeholders such as the participants and providers, be aware and encouraged to participate in any ongoing social or community events.

What makes OUR SERVICES different?

• We place significant importance on training for our support staff, which ensures our participants can be confident and comfortable with the skill level of their support.

• We encourage participants to have a say and provide preferences relating to the support staff working in our Community Care. This might include providing preferences for the ideal support staff, having meet and greets and providing ongoing feedback relating to suitability.

• We have a direct point of contact for all participants at Sublime Care; a Service Coordinator is allocated depending on the individual’s needs and preferences; this role can provide ongoing support and facilitate a coordinated service that promotes quality and inclusion.
