Sublime Care as a registered High-Intensity Support
provider:
Sublime Care specialises in High Intensity supports for personal care and community participation. As a registered provider of High-Intensity support services, our workforce has competent skills to provide support under clinical guidance from our team:
• Complex Bowel Care
• Enteral Feeding and Management, including PEG feeding
Our support staff can provide these services as they have
additional training from a Registered Nurse (RN) and recognised health
professionals. This safeguards participants as the staff can skillfully perform
clinician tasks within a community setting and have the knowledge, skills and
experience to escalate any clinical situations required to promote positive
health outcomes for our participants. Sublime Care provides in-house,
personalised training for our support staff, and we encourage participants,
their circle of support and stakeholders to be involved in this process
wherever possible.
Community Care as your NDIS Core Support Choice
Our Community Care Services are a variety of support that helps a participant’s daily living activities. It falls under the category of NDIS Core Support Budgets that may be funded in an NDIS plan that aims to provide individual support to participants with everyday activities, support their current disability-related needs and work towards their goals.
What makes OUR SERVICES different?
• We place significant importance on training our support staff, this ensures our participants can be confident and comfortable with the skill level of their support.
• We encourage participants to have a say and provide preferences relating to the support staff working in our Community Care. This might include providing preferences for the ideal support staff, having meet and greets, and providing ongoing feedback relating to suitability.
• We have a direct point of contact for all participants at Sublime Care; a Service Coordinator is allocated depending on the individual’s needs and preferences; this role can provide ongoing support and facilitate a coordinated service that promotes quality and inclusion.
